Hello friends 

   We can use this rubric directly when a patient's anxiety increase when he wakes after sleep.

Metaphorically we can apply this rubric when patient have some worry about his disease and was not serious but when something he saw or listened or someone said terrible related his disease then he come to doctor.

Mean patient was sleeping(not take any action  for his suffering) when when someone say terrible thing related to his suffering he wakes and feel seriousness(anxiety) for his problem and come to the doctors.

Example:

Patient had ulcer  in the mouth due to which he had some concern when someone said that xyz had like this ulcer in the mouth ,on examination it was found cancer then patient anxeity increased(aggravated).

Direct is rubric to tab use kar sakte hain jab patient ke jaagne par uski ghabrahat(anxeity) increase ho jati hai.

Metaphorically rubric ko tab use kar sakte hain jab patient apni suffering ko lekar thoda chintit(concern) hai lekin kisi vajah se uspar dhyan nahi diya(sleeping)lekin jab woh apni problem se related bahut bhayanak/darawna(terrible) dekhne sunne ya kisi ke kahne par turan doctor ke pass aata hai (wakes- means take action)kyonki problem ki seriousness jankar uski ghabrarat badh jati hai. (anxeity aggravate) 

Example:

Kisi patient ko mouth me chhale(ulcer) the sochta tha ki time nikalkar doctor ko dikhana hai lekin jab kisi ne kaha ki yah cancer ho sakta hai.xyz ko bhi aise hi chhale the  ,jaanch hone par cancer nikla bechara bhari jawani me chala gaya.yah sunkar ghabrakar turan doctor ke paas chala aaya.
